If you would like to proceed with making a booking, a telehealth intake session can be booked online using our booking diary below.  Parent or adult intake sessions are offered via video session (a phone appointment can be scheduled if preferred).  Assessment bookings are made with you at the end of your intake session once we have a clear idea of how many assessment sessions are needed. Please note that psychoeducational assessments do not attract Medicare rebates and are billed privately.

 

Susan's availability is updated monthly.  Sessions at other times are by appointment only. Once you have chosen an appointment timeslot, you will be asked to complete our online intake and consent forms.  These online forms will take approximately 15-minutes to complete.  Please ensure you complete all fields and continue through to the end to sign and submit the online form.

 

Our online intake form will ask you relevant questions about your child's developmental and medical history, educational history, learning concerns, and strengths and interests.  For adult assessments, our intake form will ask you similar questions, including your occupational history.

To best prepare for your intake session, please email through copies of current and previous school report cards (if available), along with any previous assessment reports from other allied health professions (i.e., occupational therapy, speech pathology, hearing or vision reports, psychologist, guidance officer/counsellor etc).

Payment Terms

Sea Star Psychology accepts payment via direct deposit only.  To secure your intake or assessment session(s), payment must be made at least 48-hours prior to your appointment. You will receive an invoice via email several days prior to your appointment with payment information.  Appointments are only confirmed once payment has been received and will not go ahead if payment has not been received by 9am the day prior to your appointment. Please provide us with 24 hours notice if you need to cancel or reschedule.​
